In calcium channel blocker overdose, is intralipid recommended according to 2023 American Heart Association Focused Update?
In this consensus guideline routine use of intralipid is not recommended. The guidelines state the following evidence “A large retrospective study did not find a benefit from ILE therapy in CCB poisoning. Experimental and clinical data suggest that ILE increases absorption of lipophilic drugs from the gastrointestinal tract, potentially worsening poisoning from oral overdose. As a result, evidence-based recommendations advise against the routine use of ILE for CCB poisoning. Whether there is a role for ILE in patients who have failed other modalities and are in cardiac arrest or periarrest is uncertain.”
