Syndrome of Irreversible Lithium-Effectuated Neurotoxicity (SILENT) is a rare and poorly understood complication of lithium toxicity, defined by the persistence of neurologic sequelae for more than two months after discontinuation of lithium. While aggressive and rapid lithium removal with dialysis has been proposed as a strategy to prevent SILENT, evidence is mixed and may even caution against over-utilization of rapid elimination methods. This presentation aims to review the current literature on SILENT and the long-term neurologic effects of lithium toxicity, as well as review proposed strategies to mitigate its risk, with particular emphasis on the role of dialysis.
